First visit to a new doctor? Here's exactly how an appointment at Live Well Medical Care works, step by step — so you can arrive prepared and relaxed.
Before your visit
Book online, call (718) 975-8702, or simply walk in. Bring:
- A valid photo ID and your insurance card
- A list of medications you currently take (or the bottles themselves)
- Any past medical records or recent test results you have
- A short list of the questions or concerns you want to discuss
Arriving a few minutes early helps with registration — and our front desk speaks Bengali, so paperwork is never a barrier.
At the front desk
Our staff registers you, verifies your insurance, and handles the paperwork — we submit everything to your insurance for you. New patients complete a short intake form; we're happy to help you fill it out in your language.
In the exam room
A staff member checks your vitals first: blood pressure, heart rate, weight and temperature. Then you meet Dr. Azmir. He reviews your history, listens to your concerns — unhurried — examines you, and explains what he finds in plain language (English, Bengali, Hindi, Urdu or Arabic, your choice).
Depending on your needs, your visit may include a physical exam, screenings, vaccinations, or management of ongoing conditions like diabetes, hypertension or kidney disease.
After the assessment
You leave with a clear plan:
- Prescriptions sent to your pharmacy, with dosage and side effects explained
- Lab tests or screenings scheduled if needed
- Referrals to trusted specialists for anything requiring advanced care
- A follow-up plan — or simply advice on your next routine check-up
