How do I make an appointment at Live Well Medical Care?
Fill out the appointment request form on this website or call us at (718) 975-8702. Our front desk will call you back to confirm your date and time. Walk-ins and same-day appointments are also welcome.
What should I bring to my appointment?
Please bring a valid photo ID, your insurance card, a list of the medications you currently take, and any past medical records you have. It also helps to note down your questions or concerns beforehand.
Do you accept walk-in appointments?
Yes — walk-ins and same-day visits are welcome. Booking ahead reduces your wait time, but if you need to be seen today, come in or call us first.
Which insurance plans do you accept?
We accept most major plans, including Medicare, Medicaid, Healthfirst, MetroPlus, UnitedHealthcare, Oxford, Molina, MagnaCare and Humana. Our staff handles the paperwork and submits everything to your insurance for you — call us if you don't see your plan listed.
Do the doctor and staff speak Bengali?
Yes. Dr. Azmir and our front-desk staff speak Bengali, and the doctor also speaks Hindi, Urdu and Arabic in addition to English — so you can describe your health concerns comfortably in your own language.
What kind of preventive care services are available?
We offer annual physical exams, vaccinations, blood pressure checks, diabetes screening, cancer screenings and routine lab work — so problems are caught early, before they become serious.
