When you renew your driver license or apply for a new one, the New York State DMV requires proof that you have passed a vision test. Live Well Medical Care makes it quick and easy: we test your vision and complete the DMV Eye Test Report (Form MV-619), which you can submit with an online, mail-in or in-person renewal.
The test takes only a few minutes. Walk in any time during opening hours — no appointment needed.
What's included
- Snellen chart vision test (the DMV minimum is 20/40)
- Testing with or without your glasses or contact lenses
- Signed and completed DMV Eye Test Report (MV-619)
- Report valid for 12 months from the date of the exam
- Referral to an eye specialist and guidance on next steps if you don't reach 20/40
- Walk-ins, no appointment needed
Renewing online or by mail
To renew online or by mail, the DMV needs proof of a vision test. We complete your MV-619 and hand it directly to you — per DMV instructions the report goes with the patient, not in the mail. Before you submit, check that your name and license number on the form are correct.
What to bring
Your current driver license or the DMV renewal notice, and your glasses or contact lenses if you wear them to drive. We supply the form.
If you have an eye condition
If your corrected vision is below 20/40, the DMV has a separate process (Form MV-80L) that may require an evaluation by an eye specialist. We'll explain the next steps and refer you if needed.
