Applicants for a new TLC driver license (Medallion, Street Hail Livery and For-Hire Vehicle) must have a licensed physician examine them and complete, sign and stamp the TLC's Driver Medical Form. Live Well Medical Care provides this exam for Uber, Lyft, yellow cab and livery drivers across Brooklyn.
Our Church Avenue clinic has served Brooklyn's taxi and rideshare drivers for years. Your form is completed correctly and handed to you the same day, so your application keeps moving.
What the exam includes
- Complete physical exam to the TLC fitness standard
- Vision check
- Blood pressure and heart check
- Medical history review — epilepsy, vertigo, heart conditions and more
- TLC Driver Medical Form signed and stamped by the physician
- A copy of the form for your records
Timing matters
The TLC requires that your medical exam is dated no more than 90 days before you submit your application, and the form must be submitted within 90 days of applying. Whether you are preparing your application or have already submitted it, we can see you quickly.
What to bring
A government photo ID, your TLC application details if you have them, glasses or contacts if you wear them and a list of your current medications. We keep the TLC form on hand — no need to print it.
We speak your language
Many of Brooklyn's TLC drivers speak Bengali, Hindi or Urdu. So do our doctor and staff, so you'll fully understand the questions on the form and the results of your exam.
