If you are applying for a green card (adjustment of status) or certain other immigration benefits, USCIS requires a medical examination by a designated civil surgeon, reported on Form I-693. At Live Well Medical Care the whole process happens under one roof — the exam, the required lab tests, any missing vaccines and the sealed form.
We know immigration paperwork is stressful. We explain every step in Bengali, Hindi or Urdu as well as English, check your vaccination records carefully and complete the form exactly as USCIS instructs, so nothing delays your application.
What the exam includes
- Medical history review and complete physical exam
- Vaccination record review, with any required vaccines given on site
- Tuberculosis (TB) screening with an IGRA blood test
- Syphilis and gonorrhea screening as required by USCIS for your age group
- Mental health and substance use screening
- Sealed Form I-693 for USCIS, plus a copy for your records
What to bring
A government photo ID or passport, all vaccination records you have (from any country, in any language), a list of your current medications and any previous medical reports. No vaccination records? That's fine — we can give the required vaccines here.
Quick appointments, quick turnaround
Immigration exam appointments are usually available within a few days. Once your lab results are back, we complete the form and give it to you in a sealed envelope, ready to submit with your application or hand to your attorney.
Care in your language
Our doctor and staff speak Bengali, Hindi, Urdu and English. If you have trouble reading vaccination papers or understanding any question on the form, just ask — no interpreter needed.
